Job Summary: We are seeking a Professional Registered Nurse (RN) to join our team as a Case Manager, providing high-quality patient care and managing patient cases in a fast-paced environment.

As a Professional Registered Nurse, you will possess and apply the skills and knowledge necessary for the delivery and management of patient care from admission through discharge. You will serve as a patient and family advocate while facilitating a cooperative and collaborative environment among all healthcare providers.


Key Responsibilities:

  Assess, plan, implement, and evaluate the care needs of a designated group of patients
  
  
• 
  Provide professional nursing care to patients
  
  
• 
  Manage patient cases and coordinate care with other healthcare providers
  
  
• 
  Develop and implement individualized care plans
  
  
• 
  Monitor and report patient progress and outcomes
  
  
  
  Requirements:
  
  
  • 
    
  • 
    Registered Nurse (RN) license or license eligible
    
    
  • 
    Basic Life Support (BLS) certification
    
    
  • 
    Three (3) years of Case Management experience
    
    
    
    Preferred Qualifications:

      Bachelor of Science in Nursing (BSN)
      
      
    • 
      Specialty Nursing certifications
